Settings -> Wi-Fi -> (the blue arrow at the right of you wi-fi conection name) -> press the "Forget this Network" button.
Then reconnect to your wi-fi. Go on and see if you get a IP adress from you router.. You can see it by going in troug settings, like here on top. If not, then press "rewnew" button.
It was what i did when I was faced with the same issue...

Sep 20, 2012 3:46 PM in response to rossvarrette417by criz02,I was able to fix the issue and got my personal hotspot back. I realized that I wasn't getting any cellular data on my line even tho I had the mobile share plan. I was on wifi the whole day but when I turned off my wifi I wasn't getting my email or getting on internet. so the person on customer care did some research and push some codes to my iphone and the result that i was able to surf the web but i still didn't have any luck on personal hotspot and was advised to contact Apple. However, before I contacted Apple I did a hard reset by pressing the home button and power button at once until it restarts. Then my personal hotspot came back on! Best of luck! hope this solved your issue =)

I've been on the phone all day with ATT and Apple and spend most of the morning teaching the support people where hotspot was located in iOS 6. Should have been my fist indication it wasn't going to be a good day. So, after about five differert phone calls Apple said they can't help and I can't roll back to iOS 5.
Has anyone had any luck getting the hotspot to reappear? Have done:
- Soft Reboot
- Hard Reboot
- Reset Network settings
- Changing the date ahead a few years (don't laugh - its how you get the passbook to work)
- Verified my account (ATT) has hotspot enabled
- But when you go to general/setting/cellular hotspot shows 'Set Up Personal Hotspot'

Sep 20, 2012 5:27 PM in response to rossvarrette417by criz02,So I spoke again to an AT&T customer agent to see what they've done differently to make my iPhone work to get the personal hotspot back again. Apparently they have to reset their network setting on their side for your line to make it work and not on your phone. If you have further question you can contact me at my cell at 5103864263.

I am convinced that this is a software error in iOS6. I can't believe this got past quality control at Apple. Time to look at Samsung phones.

Using ios6 on both phone and ipad, my 4s tells me it has created a hotspot for the ipad. My ipad3 can see the 4s but it could not connect. So i dont think its an issue with the carrier. It should be an issue with ios6

Yo Everybody
I managed to fix this issue few minutes ago. There a few steps to maintain it.
1. Settings>>Safari>>"Clear History" also "Clear Cookies and Data"
2. Turn Your Cellular Data Off
Settings>>General>>Cellular>>Cellular Data (OFF)
>>Enable 3G (OFF)
3. To Maintain your Wifi
Settings>>Wi-Fi>>(Your Network Name)>>HTTP Proxy(Auto) + Leave URL empty
I hope that might help you folks!
ps. if you haven't managed to find your network, after step one you might wanna restart your network settings

How to resolve (FOR AT&T Customers):
Step #1: Contact AT&T Customer Service via telephone
Step #2: Ask them to change your plan to the following plan:
$65 (5 GB Tethering Plan) - CODE: EPG3 (iPhone 4 or 3G)
Reset/Reboot device by holding power button and Menu button until Apple Logo Appears.
Apparently iOS6 does not like the 4GB Tethering plan. The only workaround (For now) is to go to a different plan until iOS6 is updated to fix the issue of not recognizing the 4GB plan.
