I own both an iPhone 4S and an iPad (3rd Generation) and I regularily make video with both of my devices and then transfer them to my PC via USB and watch them in QuickTime.

Up until September 11, 2012, I had no issues at all viewing my videos in Quick Time. Today, (September 25, 2012), I made a video on my iPhone 4S and transfered it to my PC the way I always have, and when I opened it in Quick Time, I just got a black screen with no Audio. I then tried the same thing with my iPad and got the same results.

The videos played perfectly on the iOS devices on which they were shot with. I tried uninstalling and reinstalling Quick Time and it didn't fix my issue.
