I have an iPhone 4S running iOS 6.0 and I've read the user guide, but cannot seem to get it to take a picture while recording video.

The camera is recording video. How can it take a snap at the same time?
1. Open the camera app.
2. Move the slider from in the bottom right from photo to video.
3. Begin recording your video.
4. While your video is recording, tap the camera icon in the upper right to take a photo.
5. You'll see the shutter close on your screen each time you snap a photo.
6. Your photos will be saved to your camera roll.

Just noticed you have a 4S...iPhone 5 only, sorry.
It's one of the iOS 6.0 features and listed in the user guide.
iPhone 5, only.
You know if there's a known app which do the same as iPhone 5 ? Which will work on other iPhones ?
