Tethering enabled but still no Personal Hotspot ...

SIM and account has been setup for tethering, the SIM tethers data using another handset but my iPhone won't. When I click to say 'setup Personal Hotspot' it asks me to contact my network provider. I have reset all settings, what can I do?

iPhone 3GS, iOS 6
