I phone is disabled connect to i tunes - How do i enable my phone again?

My sim pin was locked and I forgot the pin. I called my provider and they gave me a pin and I also had to change my Password PIN.

Unfortunately i forgot my new password PIN and ended up disabling the phone.

My provider said to connect the i phone to I Tunes to enable the phone but I can't see how to do it?????
