Hi, I have downloaded the free authoring tool for creating iBooks. How do I publish the iBook to the book store - is there a cost involved in this?

Also,  If I publish an iBook, can anyone access it, or can I make it only accessible by certain people?

Yes for all three (assuming you use the .ibooks format) iBooks are only available offline, you have to download them using email, a browser, or the iBooks app (from the iBookstore).

You can't use Apple's iBookstore for selective distribution.  You would have to distribute the book yourself, for example by emailing it or providing a link to a server where you store it.  If you do not use the Apple store, the book must be free of charge if you are using the .ibooks format.
