I upgraded to iPhone5 and activated Microsoft Exchange account. When iPhone asked to sync contacts I said ok and selected "delete contacts on the iPhone" to avoid duplicates. The contacts from Exchange appear in the phone section but the Contacts icon on the main page disappeared. How do I get the contacts icon back?

Are you talking about the contacts app?
Try searching in your other apps folders or swipe your home screen.
Also look in restrictions.
The last attempt is to go to settings - general - reset - reset home screen layout.

I have looked at all app screens and folders and it is not there.
No restriction on contacts.
I tried resetting the home screen layout. No change.
Is my remaining option to start over and erase everything in the phone?
try a reset first, hold down the home/power button together until you see the apple logo and then release then wait for the phone to boot back up.

I completely powered the phone down but holding the button, then swiping to turn off. Then rebooted iPhone.

Can you find it in spotlight search?
I have series of blank contacts... lots of them... How do I get rid of it??
I don't know anything about a utilities app folder. I don't see any folders on any screens.
However, if I go to the search screen and type in "contacts", it shows the Contacts app - on the right it shows Utilities. I guess that indicates Contacts is in the Utilities folder. If I select Contacts, I can see the contacts. I still don't see a Contacts app anywhere. Is it possible to locate the utilities folder and move Contacts to the home screen?
yes, when searching, click and hold the contacts (once it pops up) and you should be able to drag to the home screen
