How do I create a contact link where people can input their information to automatically through the site instead of manually emailing me?

Some basic websites have a "Contact" link which directs visitors to a page where they may fill in and send their information, if they wish to inquire about the website's product. I'm using iweb and wondering if there is a way to create this format so that the information is automatically sent, rather than asking users to email me manually.

If I'm not explaining this well, here is an example of the page I'd like to create using my iweb site:

Thank you

The easiest way is to use a form service...

This app will create forms, database and send out bulk emails...

You can also create your own form and upload a formmail script to the server to handle the info...
