Podcast episodes syncing to the wrong show

I have noticed this behavior several times and it just happened again today. I listen to a bunch of podcasts that all end up having similar guests. This time the guest was Dave Holmes. He was at one time a guest on the "Alison Rosen Is Your New Best Friend" show. I listened to that about a month ago and have since deleted that episode from iTunes. Today, Dave Holmes was a guest on "The Mental Illnesss Happy Hour" podcast. I believe in both cases the title of the episode displayed in iTunes was just "Dave Holmes." When I synced this new podcast it showed up as an episode of "Alison Rosen Is Your New Best Friend" instead of "The Mental Illnesss Happy Hour" podcast on my phone.

I am using iTunes 10.7 for OSX 10.8.2. I sync my podcasts with iTunes and use WiFi sync. My phone is an iPhone 4S running iOS6 using the Podcast app. I do not have subscriptions turned on for any podcasts.

This happened again today. This time it was an episode entitled "Duncan Trussel". Today Duncan Trussel was a guest on the "Alison Rosen Is Your New Best Friend" podcast. Duncan was also a guest on "You Made It Weird with Pete Holmes" a while ago. The title of both episodes were simply "Duncan Trussel".

I removed the "You Made it Weird" episode from iTunes a while back. I exported my iTunes Library to XML and don't see the "You Made it Weird" episode in there. In iTunes, this new episode show up under "Alison Rosen Is Your New Best Friend" and the artwork and tags are correct. On my phone, this episode is showing up under "You Made it Weird" with the "You Made it Weird" episode artwork and tags.

Same here. It totally messes up titles, descriptions, and files. I have two DIFFERENT episodes with different descriptions which play the exact same audio. In fact the titles and descriptions don't even match up to each other either. I thought it might be fixed when I upgraded to an Iphone 5, but nope. Deleting until they fix this. I like some of the features of the Podcasts app, but until they fix this they really ought to add the "get new episodes" back into Music.

People say the music app conflicts if it is also open with podcasts open. But I keep making sure it's closed and it still does it.

I am now listening to a Charlie Rose podcast episode (Doris Kearns Goodwin, Mchael Beschloss, etc.) that was downloaded in iTunes 10.7 (OS 10.7.5) on my 6th gen Nano (USB sync) yesterday. However! the title shows up in my Nano as an entirely unrelated episode and podcast. It is labelled "Roe V. Wade, at Forty" and the podcast art displayed is the show KCRW's To The Point". In iTunes both podcasts play and are labelled correctly—they were downloaded within a minute of one another.

I subscribe to both shows and download episodes regularly—both of which also get added to a Smart Playlist of about 15 or so shows. In the Smart Playlist they appear one after the other (sorted in order that they were downloaded).

While this problem occurs intermittently, there appears to be no pattern, rhyme or reason that instigates it. I have not been able to solve/understand what is occurring!
