Power off the device by holding the home and on/off buttons for 10 seconds, then power it back on.

Thanks so much. I had locked into a single app. When I restarted it was still locked into the app, but when I triple clicked the home button after the restart, I got the prompt for my password.
But what happens if you forget you password?
Power cycling the device as described should unlock it. If you forget your normal unlock PIN you will have to restore the iPad and then restore your backup.
None of these solutions worked for my wife's phone. Resetting also didn't work. We read every forum discussion on this topic of locked guided access. Since we didn't set up the lost my phone feature, we cannot even unlock it that way. So, it appears my phone is now rendered useless because of this glitch.
What happens when you power cycle? It just goes back into the guided access app?
It goes right back to where it was yes, bigtoe.
When I tried to connect it to the computer, the message is this, "iTunes could not connect to “Elizabeth” because this computer has not been trusted by the iPhone." Elizabeth is her iphone's name apparently.
All this feature has done was creat a firestorm here at home about a glitch that is apparently unfixable.
Yuck. If it is an app which you can crash somehow that seems to force it out of Guided Access. My little guy has become pretty adept at crashing Netflix.
You might try putting the iPhone in DFU mode while rebooting. It is kinda tricky. Google for "iPhone DFU" for steps, but stop short of actually restoring the phone with iTunes. Basically force reboot > get into DFU mode > reboot again. I have no idea if it will work as I don't have a device I can remove the passcode from right this second.
