Put your iPhone into Recovery Mode: http://support.apple.com/kb/HT1808
I did the steps but it didn't even do that. Do you think it might the actual usb?
1. Be sure to connect the iPhone 5 directly to your Mac as opposed to via a USB hub, even if the hub is powered.
2. If you have a USB hub connected to the Mac, disconnect the hub from the Mac.
This has worked for me.
Check if your Adobe Mobile Device USB Driver is installed, you may try to reinstall it.
I had a similar problem few days ago and this did not help though. I received error messages and the driver was unable to be updated/installed. So if your phone still doesn't show up - try the following:
I uninstalled iTunes and reinstalled it then - this should have solved the issue but didn't.
I uninstalled iTunes again and then installed an older version of iTunes and it recnognized my phone immediately.
I just had to delete my itunes library,
Try the followings if you're using Windows.
1) Close iTune and Make sure iPhone is plugged in
2) Stop "Apple Mobile Device" service (Run -> services.msc -> Look for Apple Mobile Device and stop it)
3) Uninstall "Apple iPhone" device driver (Run -> devmgmt.msc -> Portable Devices -> right click "Apple iPhone" and select uninstall
4) Start "Apple Mobile Device" service
5) Plug-in your iPhone and let Windows re-install the driver
6) Open iTune and hopefully it should detect your device now.

I have Windows 8 Pro, iTunes 10.7, Iphone 4S :-/
http://support.apple.com/kb/TS1538. Also, Apple has not stated that they are supporting Windows 8 at this time, and I have read several posts of users that are experiencing connection issues with Windows 8. You may need to check with Microsoft to see if there is support for Windows 8 and iTunes.

Since I upgraded to Mountain Lion, due to I bought an iPhone 5 -- I have never been able to get iPhoto to see my iPhone. NOTE! iTunes sees my iPhone. But then I have it set to sync via wi-fi, so that may not be the issue...
I have tried every troubleshooting tip on the web, including all these on this site, and none worked.
However, I found a simple solution that worked perfectly.
Change to a non-Apple USB cable. !!!
For weeks I could not get iPhoto to see my iPhone 5 -- then one day I forgot to charge the phone, and dropped by some little shop that sells iPhone accessories and picked up an iPhone 5 car charger. (3rd party, can't recall the brand).
The cable is white, like the Apple cable, but looks slightly different.
It works. In both USB ports. The other cable does NOT work, in both USB ports. Guaranteed every time.
The Apple cable charges my iPhone, but none of the apps can see it. The 3rd party cable - everything works as you'd expect.
Just thought I'd post this here in case it helps anyone. iCloud photo stream is great for photos, but do you have ANY idea how annoying it's been that I can't get my videos off the phone? You can't believe how frustrated I've been for weeks, and how enlightening this discovery was for me. Good luck!

Tried this but could not see 'portable devices' within the device manager. Am i being thick? Thanks for any help.

Is your iPhone still plug-in (Step 1)? You will only see it while your iPhone is plug-in.
To test, open device manager, plug-in your iPhone and see whether it appear in your device manager
If it still didn't appear, check your connection between iPhone and the Windows computer

yes still plugged in an what appears if i plug / unplug in device manager is an 'unknown device' in the 'Universal serial bus controllers' section. That's where the problem is in that at step3 you list asks me to click 'Apple iPhone' but that is not visible to me. Any dieas?

i've now resolved this by very simply using a different USB port on my desktop PC. I don't understand this stuff but anyone having these issues should perhaps try all their USB slots first.
