How would I know if my iPhone was hacked into?

Did you try to do something or give it to someone to hack into it? If you mean like an outside computer hack, that is not possible without knowing your apple ID and then is difficult to do anything.
i know someone hacked my notes in iphone, they had the note page emailed directly after any changes were made to it, i found a sent email in my sent mail. they also had hacked my mail account. do not enter sensitive material in your notepad. tell everyone you know
Stop scaremongering. If anything actually happened, they managed to get hold of the password to your iCloud account. Change it to something more secure and do not share it with anyone.

So are you saying that nobody can hack in to your iPhone without knowing your apple ID and password? Because I have had issues with this and I had a feeling that my phone was messed with when I left it in my office for a while when attending a meeting!! So if somebody has your phone physically it may happen? I even had a password on my phone but I know it was messed with because some of my apps were open and also my settings were changes??
So I think someone hacked my phone and I tried changing my password to my iCloud account but it's asking me for answers to security questions I didn't not set, what can I do now to change my password? I've received emails sent from my old email address for which I had the password to but it has been changed by someone else , ugh I'm so stressed out
