OK! I might have a fix for this. I noticed many blanks on my iPhone. They would be either completely blank or would have the generic album photo. These blanks don't behave right either. They simply don't play, or when I select a different song, the EQ icon shows as though the blank is playing. Very odd to say the least! The worst part was when playing a playlist, as the music would get stuck on the blank and would not advance to the next song. The result was that a playlist of a few hundred songs would only play and loop around 1 or just a few songs.
The way I fixed it was very tedious. Perhaps there is a more efficient way, but at least for now this worked. I noticed on my laptops music library many tracks showed a duplicate in the cloud. I don't know why that is, but it could be the result of multiple upgrades and multiple library transfer to various laptops that I had since I started using iTunes. (You may also notice that those "problematic songs" will appear grayed out and with a dashed circle if you look at your Music on your device when connected to laptop). THE FIX: I started first by erasing all songs on my iPhone, and logged out of the iTunes store (on the phone). Then I downloaded the duplicate tracks and erased the old ones. After that I re-synced the phone with all the selected songs and playlists. So far I do not see a single blank and the playlist are shuffling correctly. I also logged back into the iTunes store and have not seen any changes.
I tried other ways such as manually adding songs and selecting albums instead of artists to no avail. This fix takes time but seems to work.
I am using an iPhone 5 (iOS 10.2), Macbook Pro 10.10.5 and iTunes 12.5.1.21.
