Connection timout / invaild password

I have a serious problem, my macbook use to work fine but now the wifi is giving me problems, i cant connect to WPA Personal and WPA-PSK/TKIP wifi.

It's always giving me "connection timeout", if i use the assit me it give me "The password you entered is invalid" even though the password correct.I have formated and re-install the OS, i even upgraded from Mac OS X v10.7 Lion to OS X Mountain Lion but still not working,

It works fine on WEP and RADIUS

can anyone help me out?
