Very inconsistent volume levels vis 3.5mm cable.

I have my Macbook pro hooked up to my tv. Audio is via a 3.5mm cable. The audio volume varies from loud to extremely quiet. The volume isn't changing on the tv or the macbook itself, just the output volume is very inconsistent. Any ideas why?

MacBook Pro, Mac OS X (10.7.4)
