All music is deleted in my iPhone when I sync in iTunes

i dont know why this happened, when I add new music to my library and I sync my iPhone, all the music that I had in my iPhone is deleted and I sync again and nothing happen, any ideas on this?

MacBook, Mac OS X (10.4.11), Windows Vista
