Please help me because I hate seeing my phone like this

Can you explain exactly how you got to this point and possibly include a screen shot?
Well I really don't know how this happen but all I know is that it happen when I was at work. I figure it happen when I bought my new iPhone 5 online n was awaiting it to come to my house. Even when my new phone came in n I backed up my phone on my computer before transferring to my new phone. It's doing the same thing on my iPhone 5.
I am having problems seeing my contacts and adding contacts. All I can see is just the numbers
