I do no think this is possible using Numbers. I found some tools from Adobe (InDesign) that do this.

Apple confirmed Numbers dose not supports the feature of exporting editabel PDF
you have to make a sheet having Akrobat in mind wenn greating it then export to pdf and open in Akrobat.
there you have to select Forms / Edit / left cklick and selecte Checkbox and place box were you wanted.
if you have many off them you out off luck, very time comsuming.
better to start in Akrobat in the first place.

Hi Wayne, can you elaborate how you use Adobe InDesign to accomplish the task of adding checkboxes to pdf exported from Numbers? I hava a Numbers documents with hundreds and hundreds of checkboxes but my users do not have the Numbers App so the only way to distribute is exporting as PDF but as you know, the checkboxes exported by Numbers App cannot be "ticked".
I checked Adobe Acrobat, it seems I can only insert one checkbox at a time manually, it wouldn't even line up based on the table rows exported by numbers, so it's almost an impossible task in Acrobat.
I do not have actual experience with this... I searched using Google and found that information. I would visit the Adobe website.
