I need to update to iOS 5 or higher but concerned about losing my data. Has anyone done this and managed to keep all their data, photos, songs and documents (goodreader) on the upgraded iOS?

When you update, all your files will be deleted. Then, you have to restore the backup and sync it with the PC to recover all the data

If you have them on the PC, just connect the device to your PC and sync it on iTunes. If not, you must redownload all your apps, TV shows... > http://support.apple.com/kb/HT2519

Itunes should in fact backup all those things when you specify you want to perform a backup.

Media files synced from your computer aren’t backed up, but can be restored by syncing with iTunes.
You can also backup to iCloud if you have less than 5Gb of content to backup.

Different Backups. Backup for Update, and Stand Alone Backup.
Backup First, then Perform the Update. A Stand alone Backup, will backup all content. Then you can perform the Update, and finally restore from the full backup created.
