When I record a video on the iPhone 5 with the latest iOS, there is no sound when I playback, just hissing and static.
I can record and playback voice memos, use FaceTime and the telephone, all with no issues.

If you have a screen protector on the back either take it off or just cut it shorter at the top - the screen protector was covering my video mic hole. Its between the camera hole and the flash.

also sometimes with the screen protectors sometimes crumbs and stuff get stuck in them, try cleaning around where the mic is on the back next to the camera. its a little hole.
Honestly, if there is no case or screen protector on the thing, restore as new. If it persists, the iPhone needs to be replaced.
