There has never been a facetime icon on any iphone.
As the manual explains, open contacts, select a contact, scroll down to Facetime button

I did the same thing...there is a facetime app and I deleted mine to...if you go to apple.com and pull up the iPhone 5, you sill see that there is a FaceTime app on it...I'm trying to find out how to get mine back as well

It's the invisible one, just to the right of the Settings icon.

When you on apple.com, select built in apps under iPhone 5 and scroll down you will see the icon...no it doesn't show on the phone that ocean20 posted but take a look
First things first. Make sure the service is activated. Settings / Messages / iMessage

No mention of an APP. Like everyone is saying - go to contacts - pick one - and press the FaceTime BUTTON.
