1. If it is still covered by your Applecare plan then find the website for your local Applestore and look for the link for "genius bar" appointments. The only way to make an appointment is online.
2. If it is not covered by an Applecare then you can either make appointment as discussed above and pay Apple to fix or replace it (they choose which) or you can try a third party repair facility. I've had good luck with PDASmart -- google for their webpage.
