I have been unable to connect to AT&T's Personal Hotspot after installing 6.0.1 on my iphone 5. I went to AT&T store and they said this is a known issue by Apple. I closed my Hotspot (to stop paying for a service I cannot use) until Apple updates to correct issue.

I figured out the issue. Turn off FaceTime over cellular, restart the iPhone and the tethering works. Somehow turning on FaceTime over cellular breaks tethering.

Had the same issue turning off FaceTime on both devices and restarting fixed the problem!
I just changed to a new iPhone 5 with iOS 6.1.4 experiencing this problem. It would not work with my iPad iOS 6.1.3. Followed advice and disabled FaceTime over cellular & it works great without a restart. Thanks :-)
This works one time for me. But after that, same problem. Re-starting the phone or resetting the network setting will result in the same temporary fix even if you do not turn of Face Time over cellular. Apple knows they have a problem but will not admit it. I have lost all trust in Apple. If this is not acknowledged and fixed soon, I am off to AT&T for a Samsung Galaxy!
