iPhone 4S lens scratched

I noticed a scratch on my iPhones lens. Whenever i take pictures. I can see a few scratches on the picture. It's noticeable. I cleaned my lens. But the scratches yet show on all of my pictures. How can I remove it? I do have an apple care+ plan. So will apple replace my phone or replace the lens? What else does the plan help in?

The lens is a component of the iPhone 4S back plate. A new back plate (obviously with a new lens) costs $29 at the Apple Store. To use AppleCare+ would cost $50, so the $29 route is the better deal. Make Genius Reservation and take to Apple, show Genius the problem, you never know they may replace back for free, or at worst $29.
