I purchased mountian lion, an upgrade from OS X 10.7.x I got an error message stating Installed Failed! because a recovery system could not be created???

Unable to add image, because unable to get mac back up since purchasing and trying to intall OS X mountian lion.

Screen: Install Failed

I have done all and when I click restart, it restarts then show it is intalling OS X and when finished I get the same ERROR MESSAGE.
