You can either use iTunes gift cards to purchase items, assuming that gift cards are available in your country, or you can follow the instructions on this page for how to create an account without giving a credit card : http://support.apple.com/kb/HT2534 i.e. select a free app in the store and click on 'Create Apple ID' when 'buying' it.
