I recently upgraded my Verizon iPhone 4S to the iPhone 5. I sold and shipped my 4S. After doing so, I realized that I forgot to check/remove the SIM card. My question is, does the Verizon iPhone 4S have a SIM card and if so, does it contain any sensitive data?

Please don't say thing you don't know anything about. Since having LTE (which is a GSM tech), verizon iphone 5 HAS a sim card.
The Verizon 4S ships with a Vodafone GSM SIM installed for use when roaming internationally. It doesn't have any personal information on it. You've nothing to worry about.
