I restored my iphone 4s ios 6.0.1 couple of times, however it doesnt work at all.
Its WI-FI still grayed out.
I heard that its hard ware problem. Should I change it with another one by apple store???
and is it possible to change it with iphone 5 by paying added price???

Apple will not allow switching of iPhones with Warranty coverage service. Make Genius reservation, so you can get perfect working replacement iPhone 4S exactly like yours.

Apple will not allow changing of iPhones, even if you pay out of warranty fee of $199 for 4S, or even more. If you have Warranty or AppleCare it will be covered, with replacement 4S. If you want iPhone 5, you will have to just buy one as usual.
Follow the instructions in the article from which your question was posted.
If the issue continues, Apple will replace with same model, color, and capacity.
Apple does not provide upgrades.
If you want an iPhone 5, buy one.

and how about them! will they gimme just an iphone or full pachage? and will it be new one or wont?
They will only swap the iPhone... you will receive a replacement from Service stock which neither you or the genius should be able to tell if it is new or refurbished.
