when i want stream from iphone to aptv do i need a wifi or router?

in order to stream from your iPhone to Apple TV both devices need to be on the same LAN (network).
You do need to have WiFi router.

can i stream with aptv with out wifi router?
the way it works is you stream to Apple TV from your iPhone (or another Apple device).
Apple TV needs to be connected to your TV via HDMI cable.

You can try Personal Hotspot on iPhone and connecting Apple TV to it.
technically it should work but I have had no need to try it yet.
