Is such a thing even possible?
Should I replace my battery?

only replace the battery if it's run time is no longer satisfactory.

Of course. Use it until the battery status indicates "replace" or it no longer holds sufficient charge for your needs.
Apple claims 1000 chrge cycles, your battery should continue to work, but functioal time will shorten.

So it, should I worry about it totally crapping out on me? Or is it certainly going to be a gradual decline?
I have a 75 month old Macbook on it's original battery. The battery is still working well, and I have no worries because I have regular back ups via Time Machine. I will wait till I have to before replacing the battery, and by then it's possible the Macbook will be worth less than the cost of a new battery.
