Apple Event: May 7th at 7 am PT
My phone switches immediately to voicemail yet the do not disturb button is turned off.
This appears to be a different problem to the DND scheduling issue as of 1st Jan. 2013.
I have never used the DND facility.
Any ideas?

Have you tried rebooting the phone? You can also go into settings and try to enable and disable airplane mode to see if that works.
I tried rebooting a few times, together with turning on and off the DND function. I have just tried the airplane mode change as suggested and rebooted and still no difference, goes straight to voicemail.

Ok then, do you know how to remove the SIM card? I would suggest removing it and putting it back in. Or, you can go to your cellular carrier or Apple store and have them do it for you. (Is this AT&T?)
Also, do you "see" incoming calls at all and they just don't ring or you never get any alert at all and calls just go straight to voicemail?
Don't know how to remove the simcard, guess its best to take it to an Apple shop. I don't get any notification of an incoming call, no vibrate, nothing on the screen or anything.

Then that is what I would do as well. You could also go to an AT&T or Verizon store and have them do it for you.
If you go to your carrier you could also ask for a new SIM card while there. Wouldn't hurt.
