Notifications on my new iPhone4 don't seem to be working?

My notifications don't seem to be working when my phone is in the "Vibrate" mode? What am I doing wrong?

Also I set it up to have the LED Flashing when my alerts go off. That is not working either, yet I have LED Flashing ON.
