Music from sources other than the iTunes store can only be transferred from your phone to your iTunes library using 3rd party software such as PhoneView. To transfer your other data and establish syncing with your new iTunes library, carefully follow this user tip from wjosten: Syncing to a "New" Computer or replacing a "crashed" Hard Drive. If you're using iTunes 11 first go to View>Show Sidebar in order to follow this guide.

Not that I know of. There's Touch Copy, but it isn't free.

I've used Music Rescue successfully. I'm not sure if it is still the same, but the trial version will transfer 50 songs at a time, with no limit, just a pesky reminder every 50 songs. The paid version without the reminder is a bit cheaper than Touch Copy:
