No matter what I do, cover pages distorted

Hoping for some helpful answers... I've tried to order my photobook three different times and no matter what I do - including making chanes - the cover of the photobook comes out distorted each time with lines running through the whole page.

I talked to Apple support and they said it was an upload error. I had tried closing all other windows, updating iPhoto - all the usual suggestions - with no help.

I've made other photobooks before so I'm wonding why such a big problem now? Anyone have any ideas on how to fix this? It was supposed to be a Christmas present and due to techincal difficulties, I still haven't been able to even order it.
