How do I completely delete a book? I delete 'this copy' but it still shows up on my shelf. I want it off my shelf and off my cloud - make it go away!

You cannot delete it from icloud.
That is the point of icloud. It lets you access your entire purchase history (as long as Apple still carries the product).
