I would like to request a book title but my iTunes version is required information. How do I find the iTunes version information on my iPad?

The version of iTunes on your iPad is tied to the version of iOS that you have on it, as it can only be updated by updating the whole iOS. I'm not sure why that form requires you to answer that question as I don't think that it's particularly relevant when requesting something to be added to the store, but you could just it answer it with iTunes 10.7.
You could also try contacting the book's publisher, as ultimately they are the people who will have to grant Apple a license to sell it in your country.
