Here's a way that can happen:

On the Mac, when syncing to the iPad, the Mac's iPhoto app is involved. iPhoto has a storage mechanism known as "Events". You may have a sync rule set up in iTunes that says something like "Sync the most recent 5 events with my iPad." That is quite common, and perhaps a default (I don't remember - but it is what I do).

So if that's the case, introducing new photos to be synced may well have a new event created which would knock the sixth oldest event out of the current sync.

Bottom line: While your iPad is connected to iTunes, go to the Photo tab and check your sync rule.

Another way to import photos. You can use a USB flash drive & the camera connection kit.

Plug the USB flash drive into your computer & create a new folder titled DCIM. Then put your movie/photo files into the folder. The files must have a filename with exactly 8 characters long (no spaces) plus the file extension (i.e., my-movie.mov; DSCN0164.jpg).

Now plug the flash drive into the iPad using the camera connection kit. Open the Photos app, the movie/photo files should appear & you can import. (You can not export using the camera connection kit.)

If you are pointing iTunes to a new folder with more photos each time, the old pictures get removed becuase the folder they were from is no longer being synced. As was mentioned above, move all the photos you want on the iPad to the same folder, then sync that single folder.

Inside that folder there can be other folders which will appear as Albums.

After doing that what pictures are now on your iPad? Is it OK?

When you sync pictures using iTunes your iPad will end up looking just like the source folder for the sync.

For example, suppose your iPad has pictures A and B on it and you sync a folder with new pictures C, D, and E. This will remove pictures A and B from your iPad and it wil now only have pictures C, D, and E.

To add pictures to the iPad you must sync a folder with the complete content you want on the iPad. Not just the new pictures. In the case above one must sync all the pictures A, B, C, D, and E.

If you placed all of the photos into one folder and then you synced that one folder, then all of the photos should be in the iPad.

Let's say that you placed all photos - including the ones that were on the iPad the first time but got erased - into a folder and you named the folder My Photos. When you connect the iPad to your PC and start iTunes, click on your iPad name in the left sidebar to select it. Then click on the Photos Tab in the iTunes window on the right - at the top of the window. In the "Sync Photos From" menu - select the folder named "My Photos". Click on Apply in the lower right corner of iTunes and all of you photos should appear on the iPad Photos App in an album named My Photos.
