I get my new iPhone 5 and also new number as well.
When I sign in my iCloud on new iPhone 5, it shows 2 phone number on iMessage no matter Macbook pro or iPhone.
How can I remove the old number ?

There you can add and remove settings that may help you.

On your old iPhone, turn off iMessage or, if you created a support profile with Apple, login to that support profile & remove the phone.
I can only see add email....
Basically, i turned off iMessage from old iPhone already but it still appeal on my macbook pro and iPhone5 as the photo

Exactly, what do you mean by that? There are only three ways to remove a device that's associated with your Apple ID for iMessage. You turn off iMessage, on your phone; remove it from your support profile; or if running iOS 6.0 or higher, you erase all content & settings. I have not verified that the erase all content & settings works. This was a change Apple added to iOS 6.0 to stop the persistent association of an Apple ID, for iMessage, to a device.
I think I need to erase all content & setting as I just upgrade the old phone to iOS 6.1.
If you upgraded to iOS 6.1, you re-activated iMessage for that phone.
It's actually a more simple fix than I thought. I was able to fix this tonight using Apple's help after trying for weeks to figure this out.
All you need to do to remove a phone number you no longer have access to is to log out of Apple ID in both your iMessage and FaceTime & then change your password. Simple fix!
