Of course the phone can not deal with the busy case because it is dealing with the other call. However in the case the unanswered call the phone could reply to the caller and keep a recording of the call content just like the answerphone on a landline phone avoiding having to pay the carrier to keep the content. Maybe Apple could fit this into their iPhone development some time, and meanwhile mention in the iPhone guide that divert on busy and no reply is handled by the carrier. In the UK EE customers text VM ON to 150 to switch on the voicemail service.

The network knows that you did not answer, and, if it is provisioned correctly, the network will divert to voicemail after a carrier-determined number of rings. You can set your own outgoing message, and hear your voicemails using Visual Voicemail on your phone. But the carrier still has to set it up.
