I clicked on the Address Book icon in the dock to open it and the title across the top of the screen showed "Contacts".  I then opened Time Machine and it showed multiple overlaid address book cards called "Contact" and the front one showed zero contacts.

I stepped back in time until I came across a tab showing lots of contacts. I clicked on this tab to bring it to the front.

Then I highlighted all the addresses from A to Z  and clicked the "Restore" button in the lower right hand side of the screen.

My contacts were then all restored.

Unfortunately, they were displayed in a flat card format rather than the usual Open Book format, but at least the info was all present and correct.

I am currently seeking guidance on how to make the Address Book display as a booklet.

I hope this helps you, long ago though you asked.
