file with a question mark

During start up I get a file with a question mark. I ran Disk Utility and it said Hard Drive is fine. Any suggestions?

MacBook Pro (13-inch Early 2011), OS X Mountain Lion (10.8.2)

Restart with the Option key held down and see if the drive appears. If so, check the setting in the Startup Disk pane of System Preferences. If not, install an OS onto the drive.
