iphone 5 saying fully charged when its not?

My iphonw 5 has been fine all the way until this last week. Never dropped it.

Whenever I plug it into the charger it will charge, then if I barely touch it, it will viberate like 10 times then say fully charged. Then when I take it off, its still low battery. What is this issue? is it the phone or charger?
