Q: Trouble with TOC to epub?

I am creating a long-ish book in Pages specifically to publish as an .epub.

I'm having contents page issues,

on the pages document the contents page looks like this:

firstly, I don't undersand why they all seem to be on page 1, but more annoying at the moment is the fact that when it exports, this happens:

the 'introduction' on the LEFT is an empty page when it is tapped/clicked on,

and the real 'introduction: visionary Art' starts, instead on 'Chapter 1'
