The sleep/wake button on my phone is not working. Called apple and they said they would replace it, but I forgot to ask what address do I send it to, and what should I include with it?

You should contact Apple and arrange for a replacement.
DO NOT simply mail it back without arranging the replacement via Apple Customer Support first. IF you do, you WILL NOT get your device or a replacement back.
If there is an Apple store near you, go there and they will replace it on the spot if the issue is covered under warranty.
