the internet working perfect with 3G but if i want make a call or received a call i must shutdown 3G

Well making phone calls is a phone carrier feature, so you can question my suggestion, or you can call your carrier. Up to you.
but i think this problem appear in other country, and apple issue update for iphone 4s to solve this problem, and i was wait the update for iphone5 to solve this problem but nothing happened

Some carriers cannot support voice and data at the same time. If your carrier is one of these that would explain what you are experiencing.
my carrier support voice and data at the same time, i dont know where the problem, because if i make reset network settings, after it i can make a call, but the problem back again fas
but this problem disappear if i have a full network in other places
