How come I am now getting double screens?

I bought my Apple TV 3 a year ago. It's been working great. I turned on AirPlay from my iPhone 5 and got a double split image on Apple TV. I haven't been a me to get rid if it even with restore or unplugging my Apple TV from power source. Any ideas?
