That's caused by using the same Apple ID for iMessage on both phones. Doing this for FaceTime will also cause problems. To fix these, go to Settings>Messages>Send & Receive on her phone, tap the ID, sign out, then sign back in with a different Apple ID. Then do the same thing in Settings>FaceTime. You can continue to share the same ID for purchasing from the iTunes and App stores without any problems.

For iCloud, it the account is yours, go ahead and go to Settings>iCloud and sign back in with your ID. Then on her phone, go to Settings>iCloud and sign in with a different ID (such as the one she used for iMessage and FaceTime). Then turn her iCloud sync setting preferences to On and when prompted choose Merge to upload her data to the new account. (If she already has a separate existing account, she can just sign in with the ID for that account instead.)
