I've added my iPad and MacBook but how come all the messages I've sent and received to date from my iPhone aren't downloaded to the two devices? Individual contacts only start appearing on my iPad and Macbook when I receive new messages from them on my iPhone. What can I do to fix this?

If I understand what you are stating, it sounds like it is working as designed. Previously sent messages will not show up. Also, neither the iPad or the MB will receive standard SMS messages.
Not quite sure what you mean by "Individual contacts only start appearing on my iPad and Macbook when I receive new messages". Do you mean they aren't in your Contacts?
Why aren't iMessages from my iPhone in sync on my iPad and MacBook Pro?
