iPhone should be synced with only one computer.

If you change iPhone and you wish to recover all your data and apps from old to new iPhone you need to use the same computer used for sync. If you change computer you will not recover anithyng and delete all tha data on the new iPhone because it will sync with a computer which has no data.

You first need to sync your iPhone with the computer you will be using for the iPhone 5. If you don't have your music on it you will have to get it there somehow. This tip describes one way: Syncing to a "New" Computer or replacing a "crashed" Hard Drive. If all of the music was purchased from iTunes, or you registered for iTunes Match, you can use iCloud instead.

Once the music and other content is on the new computer (or in iCloud) you can sync the old phone to it, then transfer the content to the new phone as described here: http://support.apple.com/kb/HT2109
