Try a reset. Hold down the Power and Home button until Apple logo appears. Ignore "Slide to Power off".

If you are using an exchange mail account some admin's have that option disabled.

Check if you turned Restrictions on before, in Settings>>General>>Restrictions.

Are using the Contacts App? The "+" sign to create a new Contact is there not in the Phone app.
